Scholarship application

Oldsmar Soccer offers scholarships to families who need help with the cost of playing. Everything you tell us here is read only by the board.

The season

The athlete

Leave blank if there is none.

Parent or guardian

Leave the guardian address blank if it is the same as the athlete's.
Apartment, suite, unit, floor. Leave blank if there is none.

What you are asking for

A dollar figure, e.g. 120.00.

Past scholarships

Has this athlete received a scholarship before? Tick every season.
Leave this empty if they have not.

Your household

You do not need to send anything to prove this.
The board reads this. There is no right length, and it is kept confidential.

Proof of income

What to send

Any one of these is enough:

  • 1 month of current consecutive pay stubs (4 weekly, 3 bi-weekly, or 2 bi-monthly paystubs)
  • Printout from Social Security Administration
  • Most recent completed tax return
  • Notarized letter stating that applicant has no income

You may also attach anything else that helps the board understand your situation, such as:

  • Written request from parent/guardian (required)
  • Documentation showing receipt of assistance such as Food Stamps, Medicaid, SSI, Foster Care, WIC, etc.
  • Written recommendation by school representatives, social services representatives, family, or friend of the family
  • Documentation demonstrating an immediate financial hardship
PDF, JPEG, PNG or HEIC files, up to 10 MB each. A clear photograph of a document is fine.

Before you send

Confidentiality

Oldsmar Soccer will use the information on the application only to decide if your child qualifies to receive a partial or full scholarship for eligible athletic activities. Confidentiality will be maintained at all times. Applicants are guaranteed that personal finances will not be discussed outside of the Oldsmar Soccer Board. Coaches, assistant coaches, or volunteer helpers will not be informed of a participant's financial or scholarship status.

Would you rather use paper?

You can print a blank application, fill it in by hand, and post it or bring it to the club. A board member can also fill it in with you and send your documents for you.
